Information we need from you and your customers
To ensure your claim runs as smoothly as possible, be sure to have the right information in the claim lodgement. We have put together the following information as a guide, which should assist in expediting the claim. QBE may request further information upon lodgement of the claim.
Required information:
- A professional liability claim form with all applicable sections completed
- If the form is unable to be provided, we require the following to assess the claim:
- If available, please provide a letter of retainer or agreement with the claimant
- Outline when you first become aware of the circumstance and the claim
- Name and contact details of the third party making the claim against your customer
- Describe the claim being made against your customer and the relevant work undertaken, this can be provided in a written format via email or official company letterhead
- Outline the time and location the work was undertaken, it’s best to provide a complete overview
- Please provide your customer’s position regarding the third-party allegations
